How does trimethoprim/sulfamethoxazole (Bactrim™;Bactrim™DS; Septra® DS) result in clinically significant increases in the INR in patients on stable doses of warfarin (Coumadin®; Jantoven®) and thereby put them at significant risk for bleeding?
